Paint Your Pet /Stuffed Animal Party Agreement Form
    *** Please consider a date that will allow ample time for scheduling.  Be sure and check the webpage (www.paintnwithfriends.com) and view the calendar for available dates before selecting your choices.    All parties are booked on a first come first served basis.
     TABLES / CHAIRS / TABLE CLOTHS / STANDING EASELS
    If the event is held at your space, are you in need tables & chairs?      PWF has access to a few 6-foot tables and 12 chairs for your use for no additional charge.  If you do not need tables & chairs, we can provide table cloths for an additional $10.00.   
    We also have access to 14 sets of standing easels/table/and stools.  These are also available for an additional charge of $4 per set.  
    FLOOR COVERS / TARPS
    ​We can provide floor covers / tarps for no additional charge.  However, please understand that PWF cannot be held liable for any damages to floors.  
    Please bear in mind that PWF has special conditions for painters under the age of 18.  All painters under the age of 18 must have a minors release and waiver form completed before the event.  
    TRANSFER TO CANVAS
    Each painter must provide a clear a picture of your furry, feathery, scaly, or hairy friend, so we can transfer the pet’s image from the photo to the canvas.   The best photos of your pet are head shots with a minimal back ground.  All pictures must be emailed or sent via FB Messenger as soon as possible, but no later than 7 days before your party to allow us  ample time to transfer the images onto the canvas.  
    USE OF PHOTOGRAPHY DURING THE EVENT
    ​PWF is granted permission to use group or individual photographs or photo images taken in class for publicity or promotional purposes.  
    RESCHEDULING AND CANCELLATION
    ​We understand unforeseen events can arise. 
               RESCHEDULING:
                          There will be no fees charged if you reschedule your event with at least 10 calendar days notice.   **Reschedule date is contingent on PWF's availability.  
                CANCELLATION:
                           Should the need to cancel your event arise, Please notify us at least 14 calendar days in advance and a $50 cancellation feel will be        charged.  If PWF is NOT notified at least 14 calendar days in advance of the event’s start time a $100.00 cancellation fee will be charged.  
    WAIVER & RELEASE OF LIABILITY
    I agree not to hold Paint'n With Friends liable for any personal injury, property damage or loss of insurance.  I also agree to assume all liability for the consumption of alcohol.   (Absolutely no underage drinking is allowed and Underage drinking is grounds for immediate cancellation of the event with NO REFUNDS).   I agree to release and hold harmless Paint'n With Friends and Tomasa M. Brown from all liability incurred as a result of participation in this event and that these terms serve as a release for Tomasa M. Brown, volunteers, property owners and members of my family. 
    By selecting "I Accept" using any device, means or action, you consent to the legally binding terms and conditions of this Agreement. You further agree that your signature on this document (hereafter referred to as your "E-Signature") is as valid as if you signed the document in writing.
